What the Membership Provides

Membership allows enrolled individuals to receive:

• Preferred self-pay pricing for office visits

• Preferred pricing for limited in-house laboratory services

• Preferred pricing for select in-house procedures

• Access to pediatric, adult, and geriatric primary care

• Access to certain in-house specialty services (as available)

All services are provided within the clinical scope of PrimeCare Family Centers and are subject to medical necessity and provider availability. Membership provides access to preferred pricing only. Payment is due at the time services are rendered.

A Community Membership for Self-Pay Patients

PrimeCare Family Centers offers a voluntary Community Membership Program for individuals choosing to receive medical care on a self-pay basis. The program provides access to preferred pricing for office visits and select in-house services delivered directly through our clinic.

Services are provided as clinically appropriate and billed at the published membership rate at the time of service.

PrimeCare provides access to medical services at fixed, transparent self-pay rates for individuals and families who are uninsured. Patients are responsible for payment of all services at the time care is rendered. PrimeCare does not pay medical claims on behalf of patients, does not reimburse providers, and does not assume financial risk for medical expenses. This program is a medical access arrangement offered directly through our clinic to provide affordable healthcare services. This program does not cover hospitalizations, emergency services, or catastrophic medical events. Patients are encouraged to maintain appropriate major medical insurance coverage when possible.
